Let’s Get Back on Track is a health literacy initiative by the Rhode Island Public Health Institute, the Providence Healthy Communities Office, and the Providence Community Clinics Network (PCCN). This program provides Rhode Islanders with a one-stop guide to navigate healthcare, access resources, and take control of their health and well-being.
The initiative addresses health information gaps, particularly those affecting underserved populations during the COVID-19 pandemic. Its resource library was shaped by a three-month PCCN needs assessment survey, which explored community members’ physical and mental health, housing, food access, and communication preferences.
Aligned with Healthy People 2030 goals, our initiative promotes economic stability through financial incentives for SNAP recipients, making healthy food more accessible. Led by RIPHI and supported by PCCN partners, our experienced team is committed to delivering innovative and culturally tailored public health solutions that advance health equity.
